The rules, in short
- What counts: travel to reasonable, injury-related care — treating doctors, therapy, diagnostics, pharmacies, and exams the insurer sends you to. (Trips to exams they schedule are reimbursable essentially everywhere.)
- What you need: date, destination, purpose, round-trip miles. That's the whole log. Contemporaneous beats reconstructed — but reconstructed beats abandoned.
- How to claim: submit periodically (monthly is a good rhythm) to the adjuster, on your state's form or an itemized log, and keep copies. Silence after submission is worth a written follow-up; some states add penalties for late reimbursement.
